REQUEST FOR HIGH SCHOOL TRANSCRIPT – FAIRFIELD SCHOOLS, LANGDON, KS

NOTE: YOU NEED TO HAVE A SIGNATURE ON FILE AT FAIRFIELD SCHOOLS IN ORDER FOR US TO FULFILL YOUR REQUEST.

If you need your FHS transcripts do the following:

  1. Complete the Transcript Request Form (to tell us where to send it)

  2. Make sure we have a SIGNED Request and Consent to Release High School Transcript on file (it gives your permission for us to send your records to places you designate or request)

    1. If we do NOT have a SIGNED Request and Consent to Release High School Transcript on file, you will need to print the PDF form and then fill it out and mail, email or fax to the school. Click here for a printable PDF form. 

    2. Complete the SIGNED request and

      • Email to  records@usd310.org or

      • Fax it to 620-596-2835 or

      • Mail the completed request to
